Are fuel suppliers and mechanics in cahoots?

I am rather sceptical. I realise that mis-fueling is costly to repair and there are colour coded pumps to try and prevent this. What about people who are colour blind or distracted and erroneously put the wrong fuel in their vehicle?
Why not have a triangular donor and receiver connection for Diesel and continue with the round nozzles for petrol? Is this answer to the problem so obvious that it is stupid to ask (which is probably why I am asking – being thick!) or are people making money out of others misfortunes?
Adapters could be manufactured and sold for a few pennies to fit to nozzles and fuel tanks and this can save a lot of money all round. What expert has a solid argument as to why a shaped nozzle that cannot fit into the wrong inlet has not been done already? After all, they make shape hole toys for babies who learn to put the stars, squares, triangles etc into the corresponding holes!
Diesel vehicle repairs can surely be prevented using a shaped nozzle system.

Diesel fuel nozzles are larger and don’t fit into the guide plate that is on newer cars fuel necks. You can however still accidentally put gasoline into a diesel fuel tank and damage that vehicle.
